What is the School Re-entry Program?
For over thirty years, Brooks School Re-entry Program has provided a continuum of school transition and support services for children and adolescents at Brooks Rehabilitation Hospital and Brooksโ outpatient clinics. Our School Re-entry specialists serve as liaisons, collaborating with the childโs family, their medical team and their school in order to maximize academic success. School Re-entry services are customized to meet the unique needs of each child. For children recovering from a disabling injury or illness, our School Re-entry specialists work with the various school districts and school personnel to minimize lost academics while coordinating a safe and successful transition back to school.
The childโs eventual return to school is an integral part of their rehabilitation journey and we strive to make it a positive experience for the child and family. For our outpatients, School Re-entry works in partnership with the family, medical team and the school to ensure needed educational services and supports are provided. Whether through attendance at a school meeting or meeting with classmates to discuss expectations for the childโs return to school, School Re-entry seeks to offer individualized support relevant to the childโs disabling injury or illness. Some of School Re-entryโs additional services may include but are not limited to: referrals and recommendations for Exceptional Student Education or 504 Plans, early intervention referrals for ages birth through three years of age, and providing information to college students on how to obtain accommodations. The School Re-entry Program is here to help and our services are free and forever.
Inpatient
When you first arrive at Brooks Rehabilitation Hospital, a School Re-entry specialist will meet with you and your child to assess his or her educational needs. They will then work closely with you, your childโs school and our medical team to coordinate school services throughout the hospital stay and following discharge.ย Our goal is to minimize lost academics and to maximize your childโs safe and successful transition back to school. We believe your childโs successful school re-entry is an integral part of their total rehabilitation.
Services offered by Brooks School Re-entry Program include:
- Attendance at team conferences and family conferences to ensure needed school-related information is shared
- Coordination of school services while in the rehabilitation hospital and after discharge, including referrals for Hospital/Homebound, ESE services or 504 Plans
- Participation in school meetings
- Providing verbal and printed information to school personnel to assist with school planning
- Providing verbal and printed information to parents on topics related to their childโs educational needs
- Meeting with classmates to provide appropriate information to help them best support your childโs transition back to school
- Follow-up and monitoring of school transition and school services
- Ongoing access to School Re-entry specialists following discharge
- Referrals to early intervention programs (ages birth โ 3-years-old)
- Providing college students with information on obtaining needed accommodations.
- Providing verbal and printed information to parents on topics related to their childโs educational needs
- Advocating for your childโs school-related needs
Outpatient
The Brooks School Re-entry Program is also available to assist you and your child with any school-related issues during your outpatient care. School Re-entry specialists will meet with you and your child to assess his or her educational needs. They will then work closely with you, your childโs school and your childโs therapist(s) to address the educational concerns.
Services offered by Brooks School Re-entry Program may include:

- Referrals to early intervention programs (ages birth โ 3-years-old)
- Participation in school meetings
- Providing verbal and printed information to school personnel to assist with school planning

- Follow-up and monitoring of school services and transition
- Ongoing access to School Re-entry specialists at any time during treatment and beyond
- Assisting college students in obtaining needed accommodations.
