What Happens Next?

Thank you for referring a patient to Brooks. Depending on the appropriate level of care for your patient, a representative from Brooks will be in touch with either you or the patient to gather additional information and arrange for services.
Inpatient & skilled nursing admissions
Once the referral is received, an admissions liaison will reach out to visit the family and perform an evaluation to determine if the patient meets the criteria for admission to one of our three inpatient rehabilitation hospitals. Learn more about our hospital admission criteria.
Outpatient clinics/therapy
Once the referral is received, our Patient Access Center or clinic location will reach out to the patient to schedule an initial evaluation appointment at their preferred location. Typically, occupational therapy or speech therapy will need a referral from a physician. Through direct access, most patients can start physical therapy without a referral. Brooks requires a referral after 30 days.
Home health care
Once the referral is received, our scheduling teams will reach out to the patient or caregiver to verify insurance as well as ensure all the necessary paperwork is completed for care and physician oversight.
