How to Refer a Patient

Inpatient Rehabilitation

Patient has a decline in function, needs 24/7 medical care with physician and nurse oversight. Requires a minimum of two disciplines of therapy treatment. We accept national and international patients, as well as patients discharged to home if they fit the criteria.

Workers’ Compensation Program

Reach out to our dedicated workers’ comp team for inpatient admissions, as we are able to tailor a range of services unique to the needs of the injured worker.

Outpatient Clinics

The patient is not homebound and can travel to a clinic location. Our clinics provide expert physical therapy, speech therapy and occupational therapy for pediatrics (newborn-18), adults and geriatric patient populations.

Skilled Nursing

Patients receive short-term and long-term rehabilitation and nursing care with case manager support. Our rooms are private, and care is overseen by a PM&R physician.

What Happens Next?

Thank you for referring a patient to Brooks. Depending on the appropriate level of care for your patient, a representative from Brooks will be in touch with either you or the patient to gather additional information and arrange for services.

Inpatient Admissions
Once the referral is received, an admissions liaison will reach out to visit the family and perform an evaluation to determine if the patient meets the criteria for admission to one of our three inpatient rehabilitation hospitals. Learn more about our hospital admission criteria.

Outpatient Therapy
Once the referral is received, our Patient Access Center or clinic location will reach out to the patient to schedule an initial evaluation appointment at their preferred location. Typically,  occupational therapy or speech therapy will need a referral from a physician. Through direct access, most patients can start physical therapy without a referral. Brooks requires a referral after 30 days.
