Rescheduling or canceling an appointment
If you are a patient at one of our outpatient clinics or receiving home health services and need to cancel or reschedule your appointment, please contact your Brooks clinic or home health office. We ask that you provide at least 24 hours’ notice to avoid any cancellation fees.
Inpatient hospital or skilled nursing day of arrival
If you need assistance before arriving at one of our rehabilitation hospitals or skilled nursing facilities, please contact your admission liaison or visit our contact page to reach out to our admissions department.

Community Resources
Access a variety of support and rehabilitation options for you or your loved one after initial rehabilitation is over. Find information about our Aphasia Center, Brain Injury Clubhouse, Neuro Recovery Center, Wheelchair Clinic, School Re-entry Program and more.

Adaptive Sports and Recreation
Brooks has one of the most comprehensive adaptive sports and recreation programs for adults and kids. Free to participate and adaptive equipment is provided at no cost. Learn more and find an event that interests you on their website!

Medical Equipment Loan Closet
Through a partnership with CIL Jacksonville, Brooks can provide free medical equipment to patients and community members on a temporary loan status.

Brooks support groups
Families, loved ones and patients can gain support from others living with disabilities, learn about resources, attend events and more.

Peer Mentorship
A place for the newly disabled and their families to ask questions one-on-one with someone who successfully navigates life after injury.
