Brooks Rehabilitation Outpatient Clinic – Wyndham Lakes

Brooks Rehabilitation outpatient clinic in Wyndham Lakes in Jacksonville, Fla., provides comprehensive physical therapy services to help you restore movement, bodily function and increase your quality of life.

Preparing for Your First Appointment

Brooks will call you to set up your first appointment. During your initial call, feel free to ask if you should wear or bring a certain type of clothing.
Your insurance benefits and other financial information will be verified by the Brooks team. Please bring a copy of your insurance card and picture ID.
Please arrive at your appointment site 15-20 minutes ahead of the scheduled time to ensure all intake forms are completed.
During your intake, you will be asked questions about your symptoms, your medical and injury history, your family’s medical history and other personal topics.
The first appointment will include a detailed examination to evaluate your condition and begin to formulate goals, identify treatment milestones and determine the necessary frequency of clinic visits.
You will leave the first appointment with a set of instructions and recommendations for things to do – and not to do – at home.
You can schedule your next appointment before you leave the office.

Helen’s House

Helen’s House is a nonprofit hospitality house offering affordable short-term lodging to Brooks patients and their caregivers.

Helen’s House is conveniently located across from Brooks Rehabilitation Hospital – University Campus and University Crossing Skilled Nursing Facility on Beach Blvd in Jacksonville, FL.

Guest Requirements

Helen’s House serves patients of Brooks and their caregivers only. To qualify to stay with us, guests must have a referral initiated by a case manager or nurse liaison sent to Helen’s House.
A guest can remain eligible to stay if their loved one remains admitted to a Brooks Inpatient setting (Hospital, Skilled Nursing Facility, Skilled Nursing Unit) or is taking part in the Brain Injury Day Treatment, SCI Day Treatment or the Intensive Aphasia Center Program.
All guests and caregivers must reside outside the following five counties: Duval, Nassau, St. Johns, Clay and Baker.
Guests may stay at Helen’s House for a minimum of two nights.

University Crossings provides short and long-term rehabilitation and nursing care to patients who are recovering from a traumatic injury or illness.

Located on Beach blvd. and University blvd., right next to our inpatient hospital, we are the next step in your care journey after post-acute care.

At the Brooks Rehabilitation Aphasia Center (BRAC), we are dedicated to helping persons with aphasia achieve the highest level of recovery and participation in life.

Brooks Rehabilitation Brain Injury Clubhouse

The Brooks Brain Injury Clubhouse is a community program that serves the long-term community and vocational reintegration needs of adults who have sustained some form of an acquired brain injury including traumatic and non-traumatic injuries.

This clubhouse is the first and only acquired brain injury clubhouse in Florida and one of less than 20 worldwide.

Services We Provide

The Clubhouse helps individuals regain social, physical, cognitive and vocational abilities. Not only do members choose to participate in a variety of different activities, including:

Creative dance

Arts and crafts
Cognitive activities
Games
Community outings and more

Clubhouse members also have the opportunity to manage clubhouse operations by joining one of the clubhouse work units, helping them re-establish in the community and return to work.
