Becoming a Patient at Brooks

Better understand the documentation you may need to become a patient at one of Brooks’ care settings as well as our admissions and scheduling processes.

Outpatient services

Access skilled and knowledgeable physical therapists, occupational therapists and speech therapists through outpatient therapy clinics or specialized rehabilitation centers. Brooks also provides outpatient services through its PM&R physician group and behavioral medicine center for people who have experienced an injury or illness.

Patients interested in accessing Brooks’ outpatient services will need a referral from a provider for speech or occupational therapy, as well as for our Low Vision Center, behavioral medicine, Brooks Medical Group and driver rehabilitation evaluations.

Direct access is available for patients seeking physical therapy. You can schedule a physical therapy evaluation and receive physical therapy care at a Brooks outpatient clinic for 30 days before a doctor referral is needed.

Rehabilitation hospitals

Brooks’ rehabilitation hospitals take patients from hospitals and skilled nursing facilities located in the community, nationally and globally. We also accept patients from home, if they fit the criteria, and workers’ compensation patients.

Review our rehabilitation program requirements for our hospitals to better understand if inpatient rehabilitation is right for you or your loved one.

Patients need a referral for inpatient care. Your provider or hospital care team can provide a referral to us. Patients and their loved ones can also request care from Brooks below and an admissions team member will reach out to assist with next steps.

Most insurances cover inpatient rehabilitation. However, check the list of insurances accepted at Brooks to ensure your insurance provider is accepted. Learn more about the insurance we accept.

International patients

Brooks welcomes international patients to its rehabilitation hospitals in Jacksonville, Florida, through our Global Solutions Program.

We ensure our international patients and their loved ones are supported during and after the admission process through a team of dedicated admission coordinators.

Benefits of the program include:

  • 24-hour interpreter availability
  • Meet-and-greet transportation and an on-call international coordinator to ensure a smooth transition from arrival to departure

Private pay

The Brooks Solutions Program provides more than what managed care and insurance programs allow for inpatient rehabilitation.

Our program provides tiered options customized to meet the needs of each individual’s situation.

What to expect:

  • An interdisciplinary evaluation prior to admission to assess a patient’s specific need for rehabilitation, medical care and technology.
  • Collaboration on an individualized plan of care. The care plan is tailored to provide a unique rehabilitation experience at Brooks, based on the patient’s needs.

Home health care

Our home health division covers 23 counties in Florida with skilled nursing care, case management and physical, speech or occupational therapy care in the home.

Patients will need an order from a doctor to receive home health care. Many insurances cover home health care, including Medicare. Make sure to review the insurances we accept.

Custom care

In conjunction with home health, our custom care team of CNAs and home health aides provides light-duty care, companionship and transportation assistance.

Cost of care is not covered by insurance. VA benefits may apply.

Skilled nursing

At our skilled nursing facilities, patients receive comprehensive and skilled nursing care and a case manager to help facilitate the discharge to home or the next care setting.

A referral is needed for admission. Our admissions team will help support you through the process. Learn more about the insurance we accept.

Community programs

Access our aphasia center, brain injury clubhouse, neuro recovery centers and adaptive sports and recreation programs for continued recovery and support after initial rehabilitation is complete.

Many of our community programs are free of charge or offered on a sliding fee scale. No referral is necessary to participate.

Outpatient scheduling

If your provider submits a referral to Brooks for outpatient therapy, our patient access team or clinic location will call you to verify insurance coverage and set up an initial evaluation appointment within a few business days.

Submitting your own referral/direct access

If you have a hard copy of your referral or have direct access coverage, call our Patient Access Center to schedule an evaluation or fill out the form to request care.

Rehabilitation hospitals/skilled nursing facilities

Once a referral is submitted on your behalf or you filled out our form, an admissions liaison will reach out to the contact information provided to review initial information and determine if a scheduled bedside visit is needed:

  • A Brooks liaison will do a thorough evaluation at the bedside to determine if the patient is eligible for admission and meets our medical care criteria.
  • Your Brooks liaison will help coordinate admissions documents, medical records and insurance preauthorization, so you can understand your coverage options.
  • Your liaison will also work to coordinate transportation from the patient’s current location to one of Brooks’ inpatient hospitals or skilled nursing facilities.

Home health care

Our team will help you understand what is needed to access home health care. Once a referral is received from your provider, our account executives will work with your current facility’s case manager in the transfer of medical records, assist in completing required paperwork and check insurance coverage to ensure that you or your loved one has everything they need to receive our expert care in the comfort of home.
