| Intended discharge/transition environments | At Brooks, we strive for a discharge to a home environment with continued home health or outpatient services as needed. Occasionally individuals require additional time for recovery and an additional facility, such as a skilled nursing, is utilized on the road to recovery. |
| Medical acuity | Medical acuity is assessed for each patient individually by our admitting medical team to ensure we can meet the patient’s medical needs. |
| Medical stability | Medical stability is assessed for each patient individually by our admitting medical team to ensure we can meet the patient’s medical needs. |
| Participation restrictions | Must be able to participate in 3 hours of therapy 5 days per week. |
| Psychological status | Neuropsychological and psychological services are available for any patient’s needs. |
