Bartram Lake’s Assisted Living, owned and operated by Brooks Rehabilitation, a Jacksonville based non-profit health system, has been helping people navigate the course since we opened in 2013.

As you enter our spacious lobby, guests, family and caregivers alike can instantly get a sense of the comfort and love surrounding this community. At Bartram Lakes Assisted Living, we want you to feel at home the moment you walk through the door.

Once you become a resident of Bartram Lakes Assisted Living you will be welcomed into a large and attentive family. Our staff and fellow residents are warm, thoughtful and eager to make every day excellent. The care and services provided at Bartram Lakes are centered on nurturing our resident’s minds, bodies and soul, while providing the right balance of privacy dignity, independence, and support.

An integral part of independence for seniors is the ability to lead an active and full lifestyle. Located on our 115 acre campus, residents can enjoy the scenic landscapes and walking paths adjacent to ponds with fountains and water birds.

We also offer a rich variety of daily programs and activities to engage and entertain. So our residents can live life to its fullest. For those residents who still enjoy a good challenge, our billiard room is ready at a moment’s notice to test those critical shot-making skills. For those residents who enjoy a good mystery or just a fun read, our library offers a large selection of books and reading adventures for every taste.

Mealtime is when most families connect and the same is true in our fine dining room. With open seating to accommodate personal preferences, Bartram Lakes residents enjoy a variety of fresh food options served by professional wait staff three times a day. Featured specials and live action stations, keep mealtime fun and nourishing. When those special events pop up Bartram Lakes can provide a private dining room along with the wait staff and culinary experts to help make the experience memorable.

And because health and wellness take top priority, we provide nursing staff 24 hours, seven days a week, to oversee medications and clinical needs, as well as health and wellness checks for our residents.

Our staff also includes a registered dietician, pharmacy services, and individualized care plans. Our fitness center is open every day and staffed to ensure resident safety while using the equipment. We offer specialized core strengthening and muscle toning exercises to help reduce the risk of falls for our residents.

Bartram Lakes Assisted Living offers several different floor plans to best suit each resident’s needs. The Alcove provides an elegant and open design allowing for ease of mobility and maximum comfort. The Alcove features a full size bathroom, kitchenette and ample closet space. Each apartment includes electricity, water, cable television, telephone services, and Wi-Fi for keeping up with email from the kids and grandkids.

In addition to our assisted living, our campus offers skilled nursing and rehabilitation. Plus, our Green House® Residences based on a nationally recognized model of senior care for those with Alzheimer’s or other dementias. This allows our residents to age in place.
