Rehabilitative Care After Recovery

Post-rehabilitative care is important for individuals to continue to achieve recovery goals. Brooks recognizes this as a key factor in supporting our patients and their families after initial rehabilitation is over. Through our community programs, we are able to provide support to individuals in the community with varying disabilities.

Many of our programs provided help patients achieve more independence and entry back into activities of daily living.

Neuro Recovery Center

Gain access to expert physical therapists and Brooksโ€™ advanced technology to continue to work towards your recovery goals after initial rehabilitation. We have a location in Jacksonville at our University campus hospital and another location in Orange Park at our outpatient clinic.

Our adaptive sports and recreation program supports individuals with all types of disabilities to achieve emotional, social and physical wellbeing through one of the most comprehensive programs in Florida.

Wheelchair Clinic

The wheelchair seating and positioning clinic at Brooks Rehabilitation Hospital offers people with mobility limitations a variety of seating and mobility options.

School Re-entry Program

Brooks School Re-entry Program has provided a continuum of school transition and support services for children and adolescents at Brooks Rehabilitation Hospital and Brooksโ€™ outpatient clinics.

Peer Mentoring Program

At Brooks Rehabilitation, we believe in the power of peer support. Our Peer Mentoring program provides a safe place where the newly disabled and their families can discuss the challenges they face, their feelings and questions with someone who successfully navigates life after injury.

Pediatric Recreation

The Brooks Pediatric Recreation Program provides youth with disabilities meaningful and accessible recreation opportunities that offer social connection, family empowerment, and community integration to enhance quality of life.
