Welcome to Brooks Skilled Nursing Facility in Bartram
Bartram Crossing is a skilled nursing facility providing short and long-term rehabilitation and nursing care to patients who are recovering from a traumatic injury or illness.
It is located on a beautiful 115-acre campus in Bartram Park along with our inpatient rehabilitation hospital and senior living facility, Bartram Lakes.

We are staffed with trained medical professionals, such as in-house physicians, licensed nurses, physical, occupational and speech therapist and more.
24/7 Care
We provide 24/7 skilled nursing care for services such as intravenous feedings, blood pressure monitoring, medication injections, post-surgical care, wound care, continence care, diabetic and oxygen management.
Service & Amenities
Our facilities offer resident and family care meetings, daily activities, food service, daily housekeeping, laundry, therapy programs, social services and more.

I spent 17 days here in Rehabilitation. The nurses, CNA, doctors were excellent. Even the people who came and got your menu order were very friendly and helpful. I can't say enough about the quality of the OT and PT staff. Here are two of my favorites: Arryn and Suzanne saying good bye when I went home.
My mother is currently in the facility and the staff has been amazing. I can’t say enough good things about the nurses, cna’s and staff overall. The facility is clean and well maintained. Keep up the great work! And thank you for being caring and compassionate!
After hospitalization I was transferred to Brooks Rehab. I was cared for by a dedicated team including Dr. Dempsey, nursing staff, CNAs, P.T., O.T., food service, cleaning service and untold others. Care was excellent!!!
