Our 160-bed hospital provides the highest quality rehabilitation and medical care for people requiring intensive therapy. Halifax Health | Brooks Rehabilitation Center for Inpatient Rehabilitation
Halifax Health | Brooks Center for Inpatient Rehabilitation, located within Halifax Health – Medical Center of Daytona Beach, is a 40-bed inpatient rehabilitation hospital that specializes in treating stroke, spinal cord injury, brain injury and complex orthopedic conditions. A full spectrum of physical and neuro rehabilitation services are provided.
Brooks Rehabilitation Hospital – Bartram Campus
This new campus includes a 60-bed rehabilitation hospital neighboring our existing assisted living, skilled nursing and memory care facilities. Similar to our University Campus location, we care for patients following brain or spinal cord injuries, stroke, or other disabling illnesses or injuries.

Skilled, multidisciplinary teams
Patients who stay at a Brooks Rehabilitation Hospital have a best-in-class team assembled to meet their specific needs. The captain of a care team is the physician who specializes in your specific diagnoses, backed up by a nursing team that takes care of daily needs and physical, occupational and/or speech therapy.
A case manager helps patients’ families and caregivers work through the discharge plan, answering questions like what a patient will need at discharge, and how the team can ensure a safe and successful transition to their next step in care. A licensed clinical social worker can also work with families regarding any catastrophic changes.
The psychology team at Brooks includes rehabilitation psychologists and neuropsychologists, who can help with adjusting to and coping with new abilities, or perform additional testing, respectively.
Dietitians, wound care specialists, wheelchair clinicians, assisted technology specialists, and menu hostesses are some of the additional care team members who might partner with a patient. If there is a need related to a patient’s diagnosis, Brooks’ care team can meet it with individualized care and expertise.
Brooks’ care team also includes specialists that our other team members can rely on. This includes consulting doctors in cardiology, nephrology, and pulmonology. This also includes the leadership team on the floor at Brooks. Every patient is given a direct line of contact so they have someone they can turn to with any questions.
State-of-the-art facilities and equipment
The Brooks Rehabilitation Hospital features state-of-the-art facilities and equipment that are used both in the standard rehabilitation process, as well as with the Hybrid Plus program. Where appropriate, patients are admitted into the Hybrid Plus program, which allows for supplemental physical therapy in addition to the 15 hours a week of required physical therapy.
We also support assisted technology training for our patients. This includes training on technology like Google Home, Alexa, the Apple HomePod, or any additional technology that can ease a patient’s transition and independence when they are discharged.

We also offer access to our Neuro Recovery Center, which is a specialized gym equipped with the most cutting-edge rehabilitation equipment and technology available. What to expect
Plan of care
Each patient’s plan of care begins at the moment of their arrival to our rehabilitation centers. The entire care team performs an individual assessment that leads into identifying patient’s and their family’s goals. Expect hands-on, interactive rehabilitation for the patient and their caregiver/family from the start.
Intensive daily therapy
At Brooks, our goal is to create an inpatient rehab environment that is working towards recovery around the clock. The type of therapy provided and intensity level varies based on each patient’s individual abilities.
For patients with a low level of responsiveness, the focus is on upright tolerance and sensory stimuli. This helps patients reengage with their environment. For patients with a higher level of responsiveness, inpatient physical therapy can include simulations like safely getting to an appointment, returning to leisure activities, and pre-work skills.
Because we work with a wide range of injuries and ability levels, we break each individual task down into its component parts to ensure our patients can safely and successfully complete them. Both patients and their caregivers/families are engaged throughout the rehabilitation process.
Weekly care team conferences
At Brooks, we know that regular team communication is crucial for consistent patient care and maximized results. That’s why patients’ care teams — physicians, physical therapists, occupational therapists, speech pathologists, case managers, and any additional team members unique to that patient — come together each week to discuss progress, barriers to discharge, and more.
Patient/family caregiver training
One of our biggest focuses at Brooks is providing the proper training and support to patients’ families/caregivers. We recognize that the life-changing injuries we treat at our rehabilitation centers don’t just affect an individual patient — they affect the whole family. We include family and caregivers in inpatient therapy from day one, to observe, to help set goals, and to ask questions. As we continue a patient’s individual care plan, family/caregivers take on more responsibility in hands-on support. When a patient’s discharge day arrives, we want everyone to feel prepared for what’s next.
Traditional apartment training
To ensure a safe transition home, we offer apartment training as part of our inpatient rehabilitation. On campus, we are able to simulate patients’ home environments, where they can practice things like getting into and out of a tub, kitchen management, or even getting up a step that’s a certain height in front of their homes. By practicing for these specific scenarios, we aim to prepare patients for their exact home scenarios they will return to.
Graduation Day
The Day of Discharge looks very different for each patient. Many of our patients return home and begin using our outpatient facilities. Others move into places like our skilled nursing facilities. To properly select next steps, we perform reassessments of the intake evaluations just a few days before release. This helps us direct patients and their families/caregivers properly, and helps us decide on necessary equipment and additional support.

When you complete your inpatient rehabilitation, there are a number of options for continued care and support. Ideally, we aim to help patients discharge home, but even if someone needs to take advantage of a skilled nursing facility, that’s an available next step in Brooks’ support system.
