Rehabilitative Care After Recovery
Post-rehabilitative care is important for individuals to continue to achieve recovery goals. Brooks recognizes this as a key factor in supporting our patients and their families after initial rehabilitation is over. Through our community programs, we are able to provide support to individuals in the community with varying disabilities.
Many of our programs provided help patients achieve more independence and entry back into activities of daily living.
Adaptive Sports and Recreation
Our adaptive sports and recreation program supports individuals with all types of disabilities to achieve emotional, social and physical wellbeing through one of the most comprehensive programs in Florida.
Driver Rehabilitation
After an injury or illness, getting independence back is important. Our Driver Evaluation and Training Program provides assessments and training for individuals with cognitive, physical or developmental issues to help determine their driving capabilities.
Neuro Recovery Center
Gain access to expert physical therapists and Brooks’ advanced technology to continue to work towards your recovery goals after initial rehabilitation. We have a location in Jacksonville at our University campus hospital and another location in Orange Park at our outpatient clinic.
Pediatric Recreation
The Brooks Pediatric Recreation Program provides youth with disabilities meaningful and accessible recreation opportunities that offer social connection, family empowerment, and community integration to enhance quality of life.
School Re-entry Program
Brooks School Re-entry Program has provided a continuum of school transition and support services for children and adolescents at Brooks Rehabilitation Hospital and Brooks’ outpatient clinics.
Support Groups
Brooks provides support groups for people living with a disability to interact and form personal connections with people facing similar challenges.
Peer Mentoring Program
At Brooks Rehabilitation, we believe in the power of peer support. Our Peer Mentoring program provides a safe place where the newly disabled and their families can discuss the challenges they face, their feelings and questions with someone who successfully navigates life after injury.
Wheelchair Clinic
The wheelchair seating and positioning clinic at Brooks Rehabilitation Hospital offers people with mobility limitations a variety of seating and mobility options.
Our Locations
Brooks services are available to patients at our three rehabilitation hospitals, 50 outpatient clinics, home health care services–spanning 23 Florida counties, and community programs.
