Plan of care

Day of Admission

Whether the trip to Brooks Rehabilitation Hospital is short or long, we understand change can be stressful. Your day of admission is about your transition to a new place and meeting a few new faces. You will meet your nurse on this date and they will work to learn more about your child so we can ensure safe, quality care for your child from the moment you enter the hospital until you re-enter the community!

First Day of Therapy

Therapy meet and greet! We will use this day to discuss and understand your goals and plan to re-enter the community. Your child will have a chance to practice important skills such as dressing, walking, eating or communicating so that our therapists can create a specific plan to prepare you for your next steps. You will also get a chance to see the facilities where you will receive your therapy services for the rest of your stay.

Throughout your stay

Your child will have at least three hours of therapy per day. It will consist of a variety of therapies based on what is most important to your child’s recovery. Every morning a schedule will be printed for you with the information on your child’s varied appointments with locations. Monitors in patient rooms, hallways and gyms portray schedules as well.

Throughout your stay, our staff will continuously have conversations with you regarding plans for your transition to the community. This will include things such as accommodations your child may need to return to school, equipment, prescriptions, future appointments to be set up, and the scheduling of outpatient therapies. We encourage you to ask us questions so you can learn as much as possible while here!

For every family, we plan specific sessions to train you on important skills that you will need to know in order to safely transition back to the community. Some examples would be getting out of bed into a chair, walking with devices, and getting in and out of the car. Every family needs to learn what works for them, so we personalize training to exactly what your child and family needs.

Beyond this, your therapist will guide you more specifically as to other things you may participate in such as our Hybrid Program, community outings, or aquatic therapy.

Final Day

Typically, you spend this morning packing up your belongings and preparing for your transition. Additional instruction will be provided in the morning if needed. Your equipment can be delivered to your home or here in the hospital before you leave.

Planning for successive levels of care

While at our inpatient facility, we are always planning with you as you are writing the pages of your own success story. You can expect to be in constant discussion about your stay and transition home with your therapy and nursing team while you participate in daily therapies. Beyond this, every week your entire care team will meet to discuss your child’s progress and take dedicated time to ensure we are on the best possible pathway to maximize your child’s potential. You will have an opportunity to sit down with the pediatrician and case manager weekly in order to learn more about your child’s care and ensure our plan is in alignment with the values and goals of your family.

Many of our patients will continue outpatient therapies beyond their inpatient stay. These therapies will be selected based on your specific goals and needs. If you stay in our system of care, you will have the opportunity to utilize similar technologies and services well beyond your time staying in the hospital. You may also be referred to some of our specialty community programs such as our Pediatric Recreation Program, Brain Injury Day Treatment Program, Spinal Cord Injury & Related Disorders Day Treatment Program (SCI/D), and Motion Analysis Center.

Outcomes

We believe every family’s journey is unique and requires individualized care. With this, everyone has different abilities by the time they leave inpatient rehabilitation. Our goal is to maximize your child’s potential through the highest level care possible. It is our expectation for ourselves that we will prepare you so that you feel ready for your transition from inpatient rehabilitation.
