Skilled Nursing Facilities
Supporting patients with 24-hour long-term or short-term skilled nursing care along with continued rehabilitation therapy.

Outpatient Clinics
Access Brooks skilled physical, speech and occupational therapy experts in one of our 50 clinics across Northeast and Central Florida.

Home Health
Get access to skilled in-home nursing, therapy or case management care after a hospital stay or initial rehabilitation is complete. Serving 23-counties in Florida.

Community Programs
Continue on your recovery journey while gaining social and emotional support with peers through group, sports and recreational activities.

Post Rehabilitation
We provide services such as driver rehabilitation and school re-entry to help patients get back to their daily lives.

Aging and Respite Care
Stay in the Brooks ecosystem of care and age in place with our Bartram Lakes senior living community.

Physician Practice
